On the left side of the form, you will see a box stating “I authorize FedEx to leave the following referenced package:”

Below, you should fill in the following information:

  • Tracking number
  • Recipient name

On the right side of the form, you will see another box stating “At the following designated place.” Here, you should tick the box indicating where you’d like the package to be left.

Options include front door, back door, other, or immediate neighbor/ building manager.

If you select the immediate neighbor or building manager option, you must provide their address.

In addition, that person must be present to sign for the package when it arrives.

At the bottom of the form, you must print your first and last name, sign, and date the form.

Do not complete the bottom portion of the form marked “FedEx use only.”
