FedEx’s claim form can be used for FedEx Express, FedEx Ground, and FedEx Freight shipments sent domestically in the US from the US to other countries, or with Canadian shipments to the US in 2023. Claim forms can be used when shipments are not received or when they’re damaged or missing contents.

What Types of Claims Does FedEx Accept?
Before filling out a claim form, it’s helpful to know exactly what types of claims FedEx accepts so you don’t waste your time.
FedEx’s Claims Online supports claims for FedEx Express, FedEx Ground, and FedEx Freight for the following shipments:
- US domestic shipments
- US shipments to other countries
- Canadian shipments to the US
To file a claim on a shipment originating from outside the US, contact FedEx International Customer Service at 1.800.463.3339.
In terms of the types of claims that are accepted, FedEx’s online platform provides options for the following types of claims:
- Shipment not received
- Shipment missing contents
- Shipment damaged
- COD payment not collected
What Do I Need to Complete a FedEx Claim Form?
You’ll need your tracking or PRO number to complete the FedEx claim form.
In addition, all claims filled with FedEx must be accompanied by documents supporting the amount of the claim.
Such documents could include things like the following:
- Original manufacturer or purchase invoices
- Estimates or invoices for repair
- Expense statements
Finally, it can be helpful to provide photos to show proof of damage and/or missing contents when appropriate.
Don’t worry if documentation and photos aren’t available when you start your claim, as you can add them at a later time.
How Do I Complete a FedEx Claim Form?
To complete a FedEx claim form for shipments that are damaged, lost, or missing contents, you can file online (most common), in writing, or over the phone.
Let’s take a look at filing a claim with each of these methods:
To file a claim online:
Step 1: Complete the online claim form
To complete the form, enter your tracking/PRO number, select your claim type, and answer questions about your claim.
Step 2: Add supporting documentation now or after you file
Add any documents or photos to your claim form that support your case. You can attach documents by dragging and dropping them from your computer to the claim form.
Supporting documents include things like the following:
- Scan of FedEx US Air bill, FedEx Ship Manager printout, and/or FedEx Ground Pickup Record
- Photos of the package and damaged contents (if applicable)
- Serial number(s) of damaged or lost merchandise
- Proof-of-value documentation (e.g. copy of the original invoice, a screenshot of an online order with proof of payment, or an itemized repair invoice)
Note that documentation is not required for claims with declared values of less than $100.

Step 3: Submit your online claim form
Review the information you’ve provided, submit the claim form, and print the case number page for your records.
Step 4: Schedule an inspection or conduct your own
FedEx will contact you if your claim requires an inspection. You can either schedule an inspection with a FedEx representative or conduct your own.
Customers are permitted to conduct their own inspections for FedEx Express and FedEx Ground shipments with a declared value greater than $100 and less than $1000.
To complete a self-inspection, download and complete the Inspection Report, attach digital photos of your package, and submit the completed report online.
Step 5: Track the status of your claim
Log in to your FedEx account to check the status of your claim.
To file a claim by mail:
Download the claim form and fill in the following information:
- Sender information
- Recipient information
- Tracking number
- Date of shipment
- Number of pieces
- Shipment weight
- Any supporting documents

Contact FedEx customer service at 1-800-463-3339. Say Claims when prompted. If filing a claim for late delivery, say Billing.
You’ll be connected with a customer service representative who will fill out the claim form for you. After, you will receive a case number for your claim.
If you need to submit supporting documentation, you can do so online or through the mail.
How Long Do You Have to Complete the FedEx Claim Form?
If your claim is related to shipment loss, damage, delay, or collect-on-delivery payment, you should file your claim within 21 calendar days of the delivery date.
For any non-delivery or misdelivery of your shipment, you should file your claim within 9 months of the shipment date.
What Happens When You File a Complaint Form at FedEx?
You should hear from FedEx within 7 to 10 business days after submitting your claim form.
When you’re contacted FedEx may tell you that they need additional documentation to process your claim.
